EnableComp Employee Health Coverage Tax Document Form 1095-C

Form 1095-C, Employer-Provided Health Insurance Offer and Coverage provides important tax information about your offer of health coverage by EnableComp.  If you were not a full-time employee or covered individual at any point during calendar year 2025, you do not receive a Form 1095-C.


To request your Form 1095-C, you can:

> Request by phone at 615-722-4037.
> Mail a request for statement to:

Be sure to include your full name and any other applicable identifying information. We will provide the Form 1095-C within 30 days of your request.
